Patient Checklist for Minor Surgery
In the lead-up to your surgery, follow this simple checklist to help ease your fears or concerns and best prepare you for your visit to Westside Private Hospital.
- Know the date, time and address for your procedure
- Ensure you have allocated the entire day for your procedure
- Arrange for a responsible adult to collect you from hospital. You will also need a responsible adult to stay with you for the first 24 hours post procedure
- Ensure that you have read and followed all instructions relevant to your procedure, including fasting times
- Get a good night’s sleep
- Shower or bathe, wash your hair and remove makeup, jewellery, body piercings and nail polish
- Wear loose, comfortable clothing to hospital and flat or low-heeled shoes
- Provide us with your medication history, especially if you’re taking weight loss medication, insulin or blood thinning medications
What to Bring to Hospital
- Procedure consent forms (if given to you by your doctor)
- Medicare card
- Health fund details
- Relevant x-rays, pathology, cardiology, or other medical reports
- Eftpos or Credit Card to pay your account
- Supply the name and contact number of the responsible person collecting you
- Personal/Next of Kin details
Please Bring (if applicable):
- Medications to be taken during your hospital stay (eg. insulin, asthma puffer, eye drops)
- Prescription glasses (disposable contact lenses must be removed before your procedure)
- Referral Letter
- Walking aids
- Hearing aids
- WorkCover letter
- Enduring Power of Attorney or Advance Health Directive
- Pension/DVA cards
- Benefit details (e.g. Healthcare card or pension card)
- Information your doctor supplied to you regarding implantable medical devices (e.g. prosthetic and disposables)
Please Do Not
- Do not smoke or chew gum
- Do not wear make-up, false nails or nail polish
- Do not wear contact lenses to hospital (if they are the type that require removal at night time)
- Leave jewellery and valuables at home (wedding band is permitted)

Prepare For When you Arrive Home Following Surgery
- Make sure you have any necessary pain relief on hand
- Ensure you have a responsible adult staying with you overnight
- Be prepared to have some time off work. Please notify our hospital if you require a medical certificate. We will ensure your surgeon completes one for you.
