- Indemnity Insurance Certificate
- Curriculum Vitae (CV)
- Three professional references
- Working with Children Check (WWC)

Approval Process
Your application will be reviewed by our Medical Advisory Committee (MAC). Temporary accreditation may be granted, pending full approval at our next MAC meeting.

Credentialing Validity
Approved specialists are credentialed for three years, after which time they must reapply for clinical privileges.
